Horizon's campus model brings six critical service areas together so people can access everything they need without navigating a fragmented system. All services below are currently in development as we build partnerships and infrastructure.

Housing & Case Management

Stability starts with a roof and a plan.

"You can't address someone's mental health when they're wondering where they'll sleep tonight."

Housing is not one of the services Horizon offers — it is the foundation everything else is built on. Without stable housing, healthcare appointments get missed, job training becomes impossible, and children can't focus in school. Every other service on this campus exists to support what housing makes possible.

The Housing Pathway

Four phases. One continuous path.

Crisis Stabilization

The first phase is getting someone off the street or out of an unsafe situation and into a stable, safe environment. This isn't just about a bed — it's about immediate safety, basic needs, and beginning to build trust with a case manager who will stay with them through every phase.

Transitional Housing

Once stabilized, residents move into transitional housing on campus — a structured environment with support services, life skills programming, and regular case management. The goal is not just to house someone temporarily, but to use this time to build the skills and resources needed for permanent stability.

Permanent Supportive Housing

For individuals and families with complex, long-term needs, Horizon will offer permanent supportive housing — stable, long-term housing paired with ongoing case management and access to all campus services. This model has the strongest evidence base of any housing intervention for chronically homeless individuals.

Independent Living Transition

For residents ready to move into independent housing in the community, Horizon provides transition support — help finding and securing housing, financial literacy coaching, connection to community resources, and follow-up case management to prevent a return to crisis.

Case Management

What case management actually means here.

Case management at Horizon is not a check-in form and a referral list. Every resident is assigned a dedicated case manager who knows their name, their history, their goals, and their barriers. That case manager coordinates across every service on campus — medical, mental health, education, employment, childcare — and advocates for the resident at every step.

One dedicated case manager per resident

No handoffs between departments. One person who knows your full situation and stays with you.

Individualized stability plans

Every resident builds a plan with their case manager — specific goals, timelines, and milestones that reflect their actual life, not a generic checklist.

Cross-service coordination

Your case manager communicates directly with medical, mental health, education, and employment staff — with your consent — so your care is actually coordinated.

Trauma-informed approach

Every case manager is trained in trauma-informed care. We understand that the path to stability is rarely linear, and we don't give up when it isn't.

Long-term follow-up

Case management doesn't end when someone moves out. We maintain contact and support for residents who have transitioned to independent living.

Crisis response

When something goes wrong — and sometimes it does — your case manager is the first call. We respond, we don't just refer.

The Evidence

Why the campus model changes housing outcomes.

Traditional housing programs struggle because housing alone isn't enough. A person can be housed and still lose that housing within months if the underlying issues — untreated mental illness, addiction, unemployment, lack of childcare — aren't addressed at the same time. Horizon's campus model means that the moment someone is housed, every other support is immediately accessible. That's not a coincidence. That's the design.

Priority Populations

Who this serves.

Horizon's housing program is designed to serve individuals and families experiencing homelessness or housing instability, with priority given to those with the highest barriers to stability.

Chronically homeless individuals

People who have experienced long-term or repeated homelessness, often with co-occurring health and mental health conditions.

Families with children

Families experiencing homelessness or housing instability, with particular attention to the needs of children in the household.

Veterans

Veterans experiencing homelessness receive priority placement and access to veteran-specific services on campus.

Individuals leaving incarceration

People transitioning out of incarceration face significant housing barriers. Horizon provides a stable landing point and reentry support.

Youth aging out of foster care

Young adults who have aged out of the foster care system and lack stable housing or family support networks.

Survivors of domestic violence

Individuals and families fleeing domestic violence situations who need safe, stable housing and wraparound support.

Medical & Mental Health

Healthcare that meets people where they are.

Physical health, mental health, and addiction recovery are deeply interconnected — and they're all addressed on the Horizon campus. Our integrated care model means a resident can see a primary care provider, a counselor, and an addiction recovery specialist without leaving campus or navigating separate systems. Care is coordinated, consistent, and built around the whole person.

Skills that open doors. Jobs that build futures.

Economic independence is one of the most powerful paths out of instability. Horizon's education and employment programs are designed to meet people at their current level and move them forward — whether that means earning a GED, completing a trade certification, or landing a first job with real career potential. We partner with local employers and training programs to make sure our pathways lead somewhere real.

Family & Childcare

Families heal and grow together.

When a parent is trying to stabilize their life, childcare isn't a luxury — it's a prerequisite. Horizon's family services are designed to support the whole family unit, from on-campus childcare that allows parents to attend programs and work, to family counseling, youth mentoring, and recreational activities that give children safe, structured environments to thrive.

Veteran Services

Those who served deserve more than a waiting list.

Veterans face unique challenges — and unique barriers to accessing the support they've earned. Horizon's veteran services program provides specialized support that acknowledges military experience and addresses the specific needs of those who've served: benefits navigation, housing priority, peer connection, and mental health services that understand the realities of service and transition.

Community Outreach

Meeting people where they are.

Not everyone is ready to walk through a door. Horizon's community outreach program goes to where people are — with food, clothing, hygiene supplies, and portable showers — and builds the trust that eventually brings people in. Outreach is not just a service; it's the first step in a relationship that can lead to housing, healthcare, and everything else Horizon offers.

The Integrated Model

These services don't just coexist on one campus — they work together.

A case manager coordinates with the medical team. The childcare program enables a parent to attend job training. The outreach team builds the trust that leads someone to accept housing. This is what integrated support looks like — and it only works when everything is in one place.
